Port Orange, FL- A Port Orange assisted living facility will stay open, with restrictions. The owners of Grace Manor received notice on July 13 from the Florida AHCA to cease operations on July 26.

The AHCA issued an amended order, changing its ruling on Grace Manor from a license suspension to a restriction. The order to close was issued after an AHCA investigator found evidence of patient-on-patient sexual assault during a routine survey. The suspect is no longer at the facility.

The owners of Grace Manor have to implement a list of 11 conditions in order to be in compliance.

The conditions include firing the current administrator and training current and new employees to recognize elder abuse.
