State Orders Port Orange Assisted Living Facility To Close

Port Orange, FL-The Agency for Health Care Administration took emergency action on Monday to protect residents of Grace Manor Assisted Living and Memory Care. The facility is instructed to cease operations by Tuesday, July 26.  An immediate moratorium on admissions is in place.

The order comes after an investigation found a failure to properly report and document several incidents of sexual assault among patients.  A male resident was found in the rooms of different female residents on several occasions acting inappropriately.

The AHCA concluded that current conditions at Grace Manor present a threat to the health, safety, and welfare of the residents.  The facility has 30 days to appeal the order.
